Each plaque is limited to three lines of print. Each line is limited to 22 characters, including spaces and punctuation. Please fill in this form telling us how you would like your personal block to appear (print clearly in capitals). You may use initials, first name only, nickname, or first name and middle initial. For serving or retired donors, under unit, please indicate the service you wish remembered, particularly service in an airborne unit, and the years of that service such as ‘2 CDO 1977-81’. Ranks and decorations will not be shown on the plaques. Standard unit military abbreviations should be used.
